REQUIRED KNOWLEDGE, SKILLS, ABILITIES
Knowledge of:
• Database technologies, programming languages and operating systems.
• Familiarity with a range of DBMS packages.
Ability to:
• Work effectively with management to achieve desired software reporting outcomes
• Work effectively with users to train regarding appropriate data entry and reporting procedures
EDUCATION AND EXPERIENCE
Education:
• Bachelor's degree in Computer Science, Information Science, Management Information Systems, or related field preferred.
• MCDBA certification highly preferred. Experience:
• Five years of database experience.
• Extensive Microsoft Access experience required.

CARE MANAGER
The Information Center, Inc., a Taylor-based non-profit agency serving southern and western Wayne County, has an immediate opening for a Care Manager. This position reports directly to Care Management Supervisor.
Duties for this position include: Conduct in-home assessments and identifies client needs, including functional assets and deficits. Obtain information on current health problems, diagnosis, medications, treatments, and nutritional status. Re-evaluate client needs, including health and nursing needs in accordance with OSA and MSA standards. Meet with Care Managers to review and discuss client needs and to formulate person centered plans. Counsel clients and families in obtaining needed medical services and education. Assist clients with Medicaid applications when applicable. Intervene with client physicians and/or health providers and other service provider agencies as necessary to follow through on client-approved person centered plans. Contact health providers for clarification of diagnosis/treatment. Assist in obtaining services and coordinates social and health-related services provided to clients. Responsible for maintaining unrestricted licensure and completing all requirements for re-licensure.
Required Knowledge, Skills, Abilities
Geriatrics, Aging field, Disability sources, Medicare, Medicaid. Posses a current valid Michigan driver's license, proof of adequate insurance and transportation for home visits, is required.
Education and Experience
Bachelor of Social Work or Bachelor degree and related experience in social work, health care, care management, gerontology, or care planning. State of Michigan Social Work certification. One year experience in public health. Knowledge of Medicaid billing and geriatric nursing desired.
