Dear Provider,
Thank you for your interest in contracting with us to provide services to Medicaid Waiver clients in Out-Wayne County. We have developed a strong provider base for services and are interested in contracting with those businesses that are able to show they meet the Standards set by the State of Michigan for In-home services. Please be sure that your business has been licensed and in operation for at least three years before continuing.
To the right are links to materials to help you get familiar with the Medicaid Waiver program and the requirements for providers. You will also find a list of approved services that The Information Center can authorize for client care, the Standards that all providers must meet for each approved service, and a copy of the actual contract.
In order to apply as a contractor, you will need to supply us with all of the documents listed on the �Required Documentation Checklist�, a copy of the �Verification of Standards� signed by the owner, copies of current insurance coverage naming The Information Center, Inc. as an additional insured, and two copies of the completed contract. Please note that page four of the contract allows you to list those approved Medicaid services that you want to provide. You will need to write in the service and the rate, based on 15-minute units. In other words, divide your hourly rate by four to determine this rate. You should know that cost containment is a primary concern, so a competitive rate for services is essential. Access to services, consistent and reliable service delivery and service quality are other major factors that determine utilization.
Also included in this packet is The Information Center�s �Client Emergency Priority Classification System�. We are requiring that all Providers adopt this classification system. An explanation of the classification priority requirements is also included in this packet. We ask that you allocate resources in emergency situations to our clients based on this system.
Please review all of the materials closely. Please complete and/or sign all appropriate areas of the contract and return both copies to me. You should know that submitting all of the materials does not guarantee acceptance into the provider network. That determination will be made after all materials have been thoroughly reviewed. You will be notified of your status within one month of submission of all materials.
