Seniors & Disabled Services

We offer extensive services to seniors and those with disabilities. Learn more about how we can help you.

Medicaid Waiver Program
This program assists elders and the disabled plan for in-home care. Designed for those who want an alternative to nursing home, it can assist with the payment of care services at home. There are specific financial requirements to qualify for this program.

Nursing Facility Transition
NFT is a program designed to assist senior citizens and handicapped adults that are currently residing in a Nursing facility and want to transition into the community.

Caregivers Support Services
Are you providing care for an older adult or person with a disability? Let us help you! We are offering a "one-stop-shop" for caregivers. Caregivers and the adult person they are caring for are welcome to attend. There will be something for everyone! There is no charge to attend. Adult Day Services
Adult Day Services provide a daytime structured program of social and rehabilitative and/or maintenance services to functionally impaired elderly persons. Most sites will accept clients from anywhere within the area. Limited transportation to and from sites may be available. Some of these programs also offer dementia specific services.
